Delayed Diagnosis in Girls: The Women's Health Dimension

In Chapter 11 we looked at how autism can present differently in girls; in this final chapter, we'll round out Part Seven by looking at the dimensions where this difference intersects with women's health.

The Long-Term Effects of a Delayed Diagnosis

Because of the camouflaging we covered in Chapter 11, girls and women who are diagnosed late often struggle for years with questions like "why can't I socialize as easily as everyone else," "why am I this tired," and sometimes with wrong mental health diagnoses (labeled as only anxiety or depression). This delay can leave lasting negative effects on self-esteem, identity development, and mental health.

Puberty and the Menstrual Cycle

As we briefly noted in Chapter 12, the hormonal ups and downs of the menstrual cycle can have a noticeable effect on emotional regulation and sensory sensitivity (Chapter 8) in autistic girls and women. Some research suggests that in the premenstrual period, autistic traits (sensory sensitivity, reliance on routine, difficulty with emotional regulation) temporarily become more pronounced. Noticing this cyclical change and flexibly adjusting support strategies accordingly (extra rest, fewer social demands) can make this period easier.

Tip / Practical Suggestion

Consider keeping a simple journal with your teenage daughter to track her menstrual cycle and the emotional and sensory changes that go with it (see Chapter 42). This awareness can help both your daughter understand her own body and reactions, and your family respond more flexibly to needs that shift throughout the cycle.

Reproductive Health and Genetic Counseling

In light of the genetic factors we covered in Chapter 4, it's important for an autistic woman to be informed about the risk of ASD in her own children. Autistic young women reaching reproductive age, and their families, should be informed at the right time about genetic counseling and reproductive health; this isn't to create anxiety, but to allow them to make informed, prepared decisions.

Gender Differences in Access to Health Care

Research shows that autistic women report more communication difficulty, misunderstanding, and delayed diagnosis in health care than men do; this applies not only to an ASD diagnosis but also to gynecological and general health issues. Raising awareness in health care for autistic women is an issue of steadily growing importance.

Challenging behaviors are usually the expression of a need or a built-up load; "understanding the function" is more effective than "trying to stop the behavior."

A safety plan prepared in advance (elopement risk, a moment of crisis, a medical emergency) reduces panic in the moment and speeds up your response.

Long-term outcomes are enormously varied; early support matters but doesn't determine destiny on its own.

Delayed diagnosis in girls and women is an issue that intersects with many dimensions of women's health (puberty, the menstrual cycle, reproductive health, access to health care) and calls for special attention.
