When removing the focus with surgery isn't possible or isn't enough, methods that regulate the nervous system with electrical signals (neuromodulation) can come into play. The most common of these in children is vagus nerve stimulation.

What Is Vagus Nerve Stimulation (VNS)?

VNS works by placing a small, battery-like stimulator device under the skin of the chest, which sends regular, gentle electrical signals through a thin wire to the vagus nerve in the neck. Over time, these signals reduce the brain's tendency to produce seizures. The device mostly runs automatically at pre-set intervals; it can also be used with a magnet so that an extra signal can be given when a seizure begins.

Who Is VNS Suitable For?

VNS is generally used as an add-on (supportive) option for children who have drug-resistant epilepsy and aren't suitable for resective surgery (or who don't get enough benefit from surgery). One point needs to be clear: in most children, VNS doesn't end seizures completely; but in about half of patients, it can noticeably reduce how often seizures happen and how severe they are. It can also become more effective over time, speed up recovery after a seizure, and improve overall quality of life.

Placement and Monitoring

The device is placed with a short surgical procedure. Its settings are programmed painlessly through the skin at check-ups and are optimized for the child over time. The most common side effects are temporary hoarseness during stimulation, a tickle in the throat, or coughing; these usually ease over time. The device's battery is replaced every few years with a small procedure.

Other Neuromodulation Methods

In adults, and increasingly in children too, more advanced neuromodulation methods are being developed that directly stimulate specific areas of the brain or detect and respond to the onset of a seizure. This field is advancing rapidly, and more options are expected to become available for children in the future. Your epilepsy center can assess whether these options are suitable for your child.
