The Concept of an Epilepsy Syndrome

Sometimes your doctor will give not just a seizure type for your child, but also the name of an epilepsy syndrome: "childhood absence epilepsy," "SeLECTS," "West syndrome," and so on. This chapter explains what this concept means, because a syndrome name carries far more information on its own than a seizure type alone.

What Is a Syndrome, and Why Does It Matter?

An epilepsy syndrome is a recognizable "package" in which certain features occur together. The main features that define a syndrome are: the typical age range in which the seizures begin, the types of seizures seen, the characteristic findings on the EEG, any imaging findings, accompanying developmental features, and — most importantly — the expected course of the condition (its prognosis).

Being able to diagnose a syndrome is actually very valuable news. Because your doctor can then do more than just say "your child has seizures"; they can far better predict which medications will work, which should be avoided, how the condition will progress over the years, and when it may improve.

Let's Explain With an Example

The sentence "Your child is having absence seizures" gives us limited information.

But the sentence "Your child has childhood absence epilepsy" says a great deal: it usually begins in girls aged 4–10, intelligence is normal, it responds very well to certain medications, some medications must be avoided, and in a significant portion of children it goes away on its own in adolescence. This is the power of a syndrome diagnosis.

Developmental and Epileptic Encephalopathies

In some severe epilepsy syndromes, in addition to frequent and hard-to-control seizures, the epileptic activity itself can also negatively affect the child's development and learning. This situation is called a developmental and epileptic encephalopathy. In these conditions, the goal is not only to reduce seizures but also to protect development by suppressing the abnormal electrical activity the brain is constantly exposed to. West syndrome, Lennox-Gastaut syndrome, and Dravet syndrome are examples of this group; we will look at these in detail in the next section.

Not Every Child Fits Into a Syndrome

It should also be noted: not every child with epilepsy fits neatly into the mold of a recognized syndrome. Some children have seizures but cannot be given a clear syndrome diagnosis. This does not mean treatment cannot be given; your doctor still creates an appropriate treatment plan based on the seizure type and other findings. A syndrome is a helpful map; not fitting every detail of the map does not mean the journey cannot be made.
