Standard Assessment Scales: ADOS-2, ADI-R, and Vineland-3
When a screening comes back positive or clinical concern becomes clear, it's time for comprehensive, standardized assessment tools. In this chapter, we'll look in detail at three tools considered the "gold standard" worldwide — the ADOS-2, ADI-R, and Vineland-3.
ADOS-2: Autism Diagnostic Observation Schedule
The Autism Diagnostic Observation Schedule, Second Edition (ADOS-2) is the most widely used gold-standard diagnostic tool in the world, based on direct structured observation. The specialist doing the assessment chooses a module suited to the child's age and language level (Module T, 1, 2, 3, or 4) and, through standard play and social-interaction tasks (such as blowing bubbles, telling a story from pictures, or acting out a birthday party), directly observes and scores signs of social communication and repetitive behavior.
The greatest advantage of the ADOS-2 is that it gathers direct behavioral data in a standardized setting, by an experienced observer, using evidence-based scoring criteria. But it isn't enough on its own; the child being on a "good day" during the ADOS-2, or not showing their usual behavior because of the new setting, makes it essential to interpret the result together with the full developmental history.
ADI-R: Autism Diagnostic Interview-Revised
The Autism Diagnostic Interview-Revised (ADI-R) is a highly detailed, structured interview conducted with the parent or primary caregiver that complements the ADOS-2. It systematically asks about the child's early developmental history (especially behaviors around age 4 to 5) and provides detailed scoring that corresponds to each of the DSM-5 criteria. Combining the "in the moment" behavioral observation of the ADOS-2 with the developmental-history information of the ADI-R greatly increases the reliability of the diagnosis.
A Simple Analogy
You can think of the ADOS-2 and ADI-R as two complementary lenses: one answers "what do we see right now," the other answers "what has happened in the past." Used together, they reveal a more complete and reliable picture than either could provide on its own.
Vineland-3: Adaptive Behavior Scale
The Vineland Adaptive Behavior Scales, Third Edition (Vineland-3) is a tool that assesses not the ASD diagnosis itself, but the child's daily living skills — in the areas of communication, daily living skills (self-care, household tasks), socialization, and motor skills. It is administered through a parent interview or questionnaire.
The importance of the Vineland-3 comes from this: even if two children have the same ASD severity level and the same IQ score, their differences in daily living skills (for example, getting dressed on their own, managing money, safety awareness) can be very pronounced. This information plays a critical role both in educational planning and in targeting independent living skills (see Chapter 37).
Tip / Practical Suggestion
Be sure to ask for the ADOS-2, ADI-R, and Vineland-3 scores included in your assessment report, and ask your doctor or psychologist to explain what these scores mean in plain language. These scores are extremely valuable reference points if you later want a second opinion at a different center or when communicating with the school.
Other Supporting Scales
In addition to the three core tools above, other scales frequently used in clinical settings include the Childhood Autism Rating Scale-2 (CARS-2), the Social Responsiveness Scale-2 (SRS-2), and the Sensory Profile-2 (see Chapter 8). Each center may use a combination of these tools based on its own experience and resources; what matters is looking not at a single scale, but at the consistency of information coming from more than one source.
When to Seek Advice
No single scale score, on its own, confirms or rules out a diagnosis. Avoid oversimplified interpretations like "the ADOS-2 score was low, so there's no autism"; the final diagnosis is made by an experienced clinician synthesizing all the findings as a whole.
